Certified Home Nursing Solutions LLC (CHNS) provides services to individuals with developmental disabilities who are residents of Wicomico County, Somerset County, Worcester County, Dorchester County, Caroline County, Talbot County, Cecil County, Queen Anne’s County, Baltimore County, Anne Arundel County, and Hartford County. CHNS offers Day Habilitation to individuals who reside in Wicomico County, Somerset County, and Worcester County. CHNS supports individuals with developmental disabilities in achieving their goals and independence while integrating within their communities. We provide services and programs to assist with increasing their quality of life.

Day Habilitation – Supports to maximize the individual’s inclusion into the community. All supports are provided in an environment that is conducive to maintain their maximum level of independence. Individuals attend the day program Monday thru Friday, 8am to 4pm.
Personal Support – Supports to assist individuals living in their own home or family home with acquiring and building the skills necessary to maximize their personal independence.
Community Development Services (CDS) – Supports to assist the individuals in developing skills to increase their independence while integrating into the community and attending community events of their choosing. Volunteer opportunities and travel training will be supported based on each individual’s interests, abilities, and skills.
Nursing Services – Supports to promote the use of resources available in the community and build on the individuals’ existing supports network to promote positive health outcomes for everyone we serve.
Employment Services – Supports for individuals with developmental disabilities to achieve the desired employment placement of their choosing. Employment services include Job Discovery, Job Development, Follow-Along Supports, Ongoing Job Supports, Co-Worker-Employment Supports, Customized Self-Employment Supports, and Career Exploration.
Supported Living – Provide individualized supports to increase their ability to live independently in their own home in the community. Individuals will receive supports according to their individual needs, abilities, and choices.
Respite – Provide relief to the caregiver’s daily routine by providing short-term care for their loved one temporarily.
Transportation – Designed specifically to enhance an individual’s ability to access community activities, transportation services may increase independence and reduce the level of services needed.
