How Can Skilled Nursing Help?
Avoiding rehospitalization after an illness or injury is part of our mission. Our team works as an extension of the patient’s physician to make sure discharge orders are understood and followed at home. We also play a key role in preventing unnecessary readmissions by intervening early when needed.
We can begin services within 24 hours of hospital discharge to allow the smoothest transition possible back into the community. We then ensure that patients have appropriately scheduled follow-up visits with their physicians and are assessed for fall risk, depression, medication adverse reactions, and more.
